Job Summary/Purpose:

Provide Coverage for inpatient DMS as scheduled with weekends and holidays. Assist with the Diabetes Management Service (DMS) providing in-patient care for patients with diabetes and hyperglycemia. Manage care of assigned inpatient caseload within the DMS service and monitor treatment plans.  Provide outpatient care for Division of Endocrinology patients, traveling to outreach clinics as required. The Nurse Practitioner or Physician Assistant will work with physicians and team management to assist in direct patient care, case management, practice development, and implementation of policies and procedures focused on patient-centered care models.

Minimum Training and Education: Completion of an accredited Nurse Practitioner (FNP or ANP) program or Physician Assistant program.

Thorough knowledge of methods, management, principles and practices of clinical counseling and education services. Extensive knowledge of psychotherapy and patient management within a psychiatric setting. Able to evaluate counselor skill and program needs and to apply counseling theory in a clinical setting. Demonstrates and ability to collaborate in a positive, collegial manner with other disciplines.

Licensure, Certifications, Registrations: Currently eligible or licensed as an Advanced Practice Nurse in the state of SC or Licensed (or certification eligible) as a Physician Assistant by the National Board of Medical Examiners or the American Academy of Physician Assistants. Requires certification in Basic Life Support.

Additional Duties and Responsibilities: Manage care of assigned inpatient caseload within the DMS service and monitor treatment plans.  Manage care of outpatients with diabetes, history of hyperglycemia, and related conditions. Utilize appropriate clinical and laboratory data to appropriately manage patients. Utilize Practice Partner for recording patient care activities and assessing clinical data to maintain outpatient health histories. Ensure that outpatient and inpatient care documentation is within MUSC compliance and HIPAA directives. 45%

Positively represent the division through active participation to various projects and initiatives. Exhibit the highest standards of customer care and commitment according to MUSC Excellence. Advocate our expertise to other services on campus and healthcare institutions by promoting the consulting and educational activities of the Diabetes Management Service. Participate with patient care team toward mastering Joint Commission goals. 25%

Work constructively with all other members of the endocrine division (physicians, nursing and administration): Communicate treatment plans; correctly perform administrative duties such as coding and documentation; appropriately seek clinical guidance when needed. 25%

Potential precepting of MUSC NP or PA students during their clinical year. 5%

Physical Requirements:  Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Ability to be qualified physically for respirator use, initially and as required. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36" to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
