Job Summary/Purpose: The Manager, Clinical Transitions reports to the Children and Women’s Health ICCE Administrator. This role serves as the subject matter expert to lead the inpatient clinical transition to the new facility.  In conjunction with stakeholders and operational leaders, this position ensures collaboration and integration of systems, processes and structure design of new facility build. In working with the various project managers, subject matter experts and CHW ICCE and MUSC leadership, the Manager of Clinical Transitions plans, directs, and coordinates the activities for the transition to the new hospitals. This role ensures goals of the patient care partners are accomplished within prescribed time frame and funding parameters. This position facilitates the development, implementation, and evaluation of organizational strategies to improve clinical, service and financial outcomes within an evidence-based practice framework.  In collaboration with clinical and administrative ICCE and MUSC leadership, the Manager facilitates the development, implementation, and evaluation of organizational strategies to improve clinical, service, and financial outcomes within an evidence-based practice framework.

Minimum Training and Education:  Master’s degree in a professional health care discipline (e.g. Registered Nursing, Respiratory or Pharmacy); or Master’s degree in a related field and Bachelor’s degree in a professional health care discipline (e.g. Registered Nursing, Respiratory or Pharmacy).  Minimum of five years’ clinical work experience which must include two (2) years of leadership experience. Project management within acute care setting using Lean or Six-Sigma principals, management of large capital project is required. Knowledge of the ICCE patient population, health care trends, community and regional resources and service availability to these populations. Requires strong critical thinking skills. Requires demonstrated ability to establish effective collegial working relationships with diverse groups and individuals, medical staff, and other healthcare disciplines. Performs under minimal supervision with accountability for specific goals and objectives.   

Required Licensure, Certifications, Registrations: Current South Carolina or compact state licensure, and/or registration/certification applicable to clinical designation is required.
