Job Summary/Purpose: The Lead Social Worker (MSW) reports to the Manager of Case Management. Under indirect supervision, the Lead Social Worker (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This position functions as a lead to facilitate effective departmental operations. The Lead Social Worker (MSW) completes an Assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and apply discharge interventions to lower the patient risk of an undesirable event post discharge. This position assists with the discharge process and promotes a successful transition out of the hospital through coordinated efforts between the health care team members to achieve targeted patient outcomes. The Lead Social Worker bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Minimum Training and Education: 

Master's degree in a social work or social welfare program accredited by the Council on Social Work Education and three years professional social work experience in designated specialty area.

Certification in case management preferred.

Required Licensure, Certifications, Registrations: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.

(Continuous) Ability to see and recognize objects at a distance. (Continuous) Ability to maintain hearing acuity, with correction.
