The Transitional Care RN provides quality psychiatric and mental health services to patients, families, health care providers and the community. These services include individual, family and group support and counseling; serving as an educational resource about the emotional aspects of physical illness, grief, stress and end-of-life issues; and functioning as a consultant to the health care team about psychiatric and mental health aspects of care. Services are provided primarily to patients 18 years of age and older. This role partners with high risk psychiatric patients to decrease over utilization of in-patient psychiatric services.
