Disability Service Navigator - Advice and Referral Line - Multiple Positions

| The role of the Disability Service Navigator is to provide specialist coordination and advocacy for children, young people, and families whose needs cannot be effectively met through routine triage and referral, due to service or system factors and constraints.
Service navigation requires deep service knowledge, active case facilitation, and authority to escalate across agencies when services are unavailable, inaccessible or fragmented. The Disability Service Navigator will work with children and families to ensure they do not remain ‘stuck’ in the system, falling between eligibility criteria or waiting lists, by actively coordinating pathways and addressing barriers. |
