Job Duties
Responsible for accurately coding admission and discharge summaries, entering patient data, and maintaining medical records in compliance with documented disease coding standards and hospital guidelines. Performs admission, discharge, and Core Measure audits; reports findings to leadership; provides coding attestations to support reimbursement and utilization review functions; and works closely with clinical departments to ensure complete, accurate, and timely documentation. Reviews patient admission packets, interviews patients to complete admission documentation, processes and distributes required forms, retrieves previous records as needed, and monitors scanned documents for accuracy, proper indexing, and HIPAA compliance. Provides clinical documentation training to staff, tracks incomplete medical records for reporting purposes, and assists with census reconciliation and commitment hearing preparation as needed. Utilizes AVATAR, HOPs, and the Electronic Health Record (EHR) system for patient registration, documentation, data entry, and record management.
Minimum Qualifications
Knowledge, Skills, Abilities (KSA) and or Competencies required to successfully perform the work (attach Competency Model, if applicable): Significant knowledge of HIM practices and procedures; ability to review clinical records for completeness, accuracy, and compliance with regulations; experience with medical and classification software; proficiency compiling clinical data and writing statistical reports; strong knowledge and experience in administrative systems and records management; knowledge of The Joint Commission and CMS regulations; Demonstrated knowledge applying coding principles.
