The Clinical Case Manager facilitates access to all core medical services and coordinates comprehensive HIV primary care service delivery planning for patients through assessment, service facilitation, referral, advocacy, and follow-up. By working collaboratively with the patient and the health care team, and utilizing established Levels of Service Criteria, the Clinical Case Manager plans and implements with the patient a Comprehensive Medical and Health-related Services Care Coordination Plan, updates the plan as needed, documents all care and service contacts, monitors patient progress, and continually reassesses the individual's need for level of service. The Clinical Case Manager provides treatment adherence readiness counseling and treatment education and support, in collaboration with the other members of the health care team.
Internal and External Working Relationships: Close internal working relationships with members of the I.M. Specialty Services Clinic and other OSU-COM Physicians Clinics and departments to coordinate patient care. Regular, ongoing external contact with local health and social service providers and local and state community-based providers to assist patients in accessing all needed medical and social service supports.
