Are you a compassionate professional with a passion for supporting people to live independently? We’re looking for a dedicated Case Manager to join our team and play a pivotal role in delivering high-quality, home-based care to individuals through the Commonwealth Home Support Program (CHSP).

In this rewarding role, you’ll work closely with consumers, their families, and carers to create tailored care plans that empower individuals to live their lives with choice and dignity. Your expertise and compassionate approach will guide each consumer's journey towards greater independence, ensuring that they receive the right support at the right time.

As a Case Manager, you will:

  • Conduct comprehensive, person-centred assessments and negotiate Care Plans focused on enhancing consumer independence and wellbeing.
  • Regularly review and adjust Care Plans to ensure they evolve with the changing needs of the consumer.
  • Foster strong relationships with consumers, families, carers, and other health professionals to promote continuity of care.
  • Maintain accurate, up-to-date consumer records while contributing to the smooth operation of the program.

Essential Criteria

  • Excellent communication skills and the ability to collaborate within a team.
  • Proven judgement and the ability to manage your time effectively.
  • Experience with Microsoft Office and a keen attention to detail in documentation.
  • A qualification in health, aged care, or disability (Certificate III or above) is highly desirable.

  • Immunisation Risk for this position is - Category A
