About the Role

As a Home Support Assessor, you’ll be part of a compassionate and skilled team within the Single Assessment Service, providing face-to-face, home-based assessments for older Australians referred through My Aged Care. Using the Integrated Assessment Tool (IAT), you’ll work directly with clients and their carers to identify individual needs, set meaningful goals, and develop personalised support plans. Your role will be key in guiding clients toward appropriate aged care and community services, helping them maintain independence and wellbeing. With your strong communication skills and empathetic approach, you’ll ensure clients feel heard, supported, and empowered to make informed decisions about their care.

This rewarding role involves accurately entering client information in the Aged Care Assessor application and My Aged Care portal, while staying up to date with aged care pathways and service options. You’ll regularly collaborate with a wide network of health professionals, service providers, and other stakeholders to deliver coordinated and effective support. As a Home Support Assessor, you’ll work within established guidelines and be supported to meet key performance indicators set by SA Health and the Commonwealth government.
