About the Role

Join a dynamic team delivering expert care that helps people stay well and avoid hospital.
Support complex clients through assessment, triage and coordinated care.
Build strong partnerships across health, government and community services.
Bring your clinical expertise to clinic, community and integrated care settings.

• Triage and assess complex health needs
• Coordinate care across services
• Deliver expert clinical interventions
• Support hospital avoidance and early discharge
• Build resilience through restorative care
• Partner with multidisciplinary and community teams
• Manage short term case coordination where needed
• Work across Integrated Care sites as required

About You

Bring your clinical expertise, initiative and commitment to quality care. To succeed in this role, you will have:

• Registered Nurse with current AHPRA practising certificate
• Post registration experience in a relevant nursing setting
• Experience caring for older people in varied settings
• Strong clinical assessment and intervention skills
• Ability to lead, collaborate and manage changing priorities
