The Clinical Case Manager I independently provides casework services to patients and families as indicated to address the psychosocial aspects of care regarding adjustment to illness and treatment, while working collaboratively with interdisciplinary staff both internal and external to the organization.
The Clinical Case Manager I will participate in quality improvement and evaluation processes related to patient satisfaction and patient rights and serve as an information resource for the Fixel Institute of Neurological Diseases (FIND) in the Department of Neurology. Credentialing for this position requires the incumbent to be licensed as a social worker in the State of Florida. |
