Job Summary/Purpose: The Nurse Practitioner or Physician Assistant will work with physicians and team management to assist in direct patient care, case management, practice development, and implementation of policies and procedures focused on patient-centered care models. The Health Care Provider for the Transplant Service Line will use advanced clinical management, consultation, education and research to promote quality care for the specific transplant patient populations. Develops and implements clinical pathway guidelines and evaluates outcomes. Advances clinical knowledge through research based practice. Coordinates staff development and assists in orientation.

Minimum Training and Education: Currently licensed as an Advanced Practice Nurse in the state of SC or licensed as a Physician Assistant by the National Board of Medical Examiners or the American Academy of Physician Assistants. Requires certification in Basic Life Support.

Preferred Training and Education: Physician Assistant: Completion of an accredited Physician Assistant program. Currently licensed as a Physician Assistant by the American Academy of Physician Assistants. Nurse Practitioner: Requires licensure as a Registered Nurse and hold official recognition as a Nurse Practitioner by the South Carolina Board of Nursing. Licensed as appropriate in the State of South Carolina. BLS certification required. An ability to establish cooperative working relationships with diverse groups and individuals, medical staff and other health care disciplines and understanding of the consultative process. Ability to establish and maintain cooperative working relationships with a wide range of groups and individuals. Highly developed analytical skills, interpersonal skills and organizational skills. Must have emotional/mental ability to perform effectively and efficiently under pressure during stressful or emergency situations. Must maintain the highest standard of professional conduct.

Licensure, Certifications, Registrations:  Currently eligible or licensed as an Advanced Practice Nurse in the state of SC or Licensed (or certification eligible) as a Physician Assistant by the National Board of Medical Examiners or the American Academy  of Physician Assistants. Requires certification in Basic Life Support.

Additional Duties and Responsibilities:

-Identifies, evaluates, and addresses disease management and health promotion issues of the renal failure/transplant patient population in the practice while providing quality patient care. Assess the patient's clinical status and response to therapy through history, physical examination, laboratory data, and appropriate diagnostic testing and develop therapeutic plan of care. Coordinates the care of the transplant patient throughout the transplant phases; pre-transplant, intra-transplant, post-transplant. 25%

-Collaborates with transplant team members to provide the most cost-effective and appropriate care for the transplant populations. Ensures systematic approach for the plan of care in collaboration with other disciplines to meet patient and family needs. Maximizes awareness and utilization of resources and services among patients, families, and staff. 25%

-Directs the day to day patient care activities of areas of responsibility utilizing a multi-disciplinary approach and the nursing process; evaluates the effectiveness of nursing care actions. Possess leadership. Facilitates negotiation and conflict management skills. Initiates and leads multi-disciplinary conferences and committees. Develop and implement clinical pathways and evaluate outcomes of clinical pathway guidelines to promote quality care. Advances clinical knowledge through inquiry and research-based practice. Participates in the evaluation, application and implementation of research. Participates in the development and implementation of programmatic changes. Participates in staff meetings. 20%

-Provides leadership in quality assurance activities. Evaluates patient population for trends and risk factors and recommends innovative strategies to improve quality of care. Favorably affects cost, quality and outcomes of services. Negotiates effective practice approaches. Leads collaborative initiatives with all disciplines to identify systems and processes to improve patient outcomes. 5%

-Acts as clinical expert for the renal failure/transplant program. Provides clinical guidance and expertise to others within area of specialty. Educates colleagues and other disciplines within area of clinical practice. Assists others in the advancement of clinical knowledge, critical thinking skills and problem-solving techniques. Assists with staff development. 10%

-Works with Clinical Nurse V to improve program systems and efficiency. Initiates maintenance and upgrading of patient education material. Initiates and plans in-service for the floor and unit nursing staff. Assumes project management responsibilities. 5%

-Assumes responsibility for professional development of self and staff. Develops own professional role in collaboration with immediate manager and collaborating/supervising physicians. Maintains CCTC or ONC. Obtains 20 contact hours per year. Maintains membership and actively participates in at least one professional organization. Institutional, local, and/or national committee leadership. Remains current with literature as demonstrated by presentation(s) to staff and incorporating new knowledge into clinical practice. 5%

-Serves as liaison between the renal failure/transplant team and outside agencies. Works closely with renal failure/transplant marketing and outreach to maximize public and professional awareness of transplantation. 5%

-Potential precepting of MUSC PA/NP students in their clinical rotations.

Physical Requirements:  Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Ability to be qualified physically for respirator use, initially and as required. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36" to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
