Position for busy Out-patient therapy clinic working alongside PTs and OTs. Clinic primarily sees Ortho population but may include others as well such as Spine, rheumatology, general medical, chronic pain, post op patients for all joints, and a wide age range of patients. Physical Therapist Assistant II works under the supervision of a licensed physical therapist to assist with a patient’s treatment program in accordance with federal guidelines and state practice acts. The PTA II assists in the provision of screens, collection of assessment data and direct treatment to a general rehab caseload in single or multiple sites under the direction and supervision of a Physical Therapist in accordance with state and national practice guidelines. The PTA II participates in the clinical team process and participates in clinical program development. PTA II also provides teaching and training to caregiver staff, and functions as a resource to the PTA I.

This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

The Complex Care Coordinator will assist with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This position will work with patients with substance use disorders admitted to MUSC med/surg units and help to facilitate smooth transitions of care and linkage with substance treatment after discharge. This role will involve working as a team with Consult Liaison Psychiatry Service and a peer navigator to provide inpatient addiction services and linkage to outpatient substance treatment. The role will involve coordination of care and communication with outpatient treatment centers. This position identifies risk factors and barriers to care that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge, bridging the discharge to outpatient provider with the patient and caregiver to address issues and questions. Under guidance of Emergency Department (ED) Project Manager, the Coordinator will play a vital role in the success of the ED-Initiated Buprenorphine Project. This role will provide peer navigator services, build a professional relationship with the Peer Navigators (PNs), and empower PN success in the ED. This position is responsible for the day-to-day performance of the ED MAT project and the PNs that are assigned to this project. The Coordinator will be open to feedback on staff performance issues related to the PNs from leadership, will work through process for correction if concerns arise, and will collaborate in the navigator’s evaluation and continued employment. Geographic responsibilities include MUSC Charleston as well as other MUSC locations.
MUHA GB81 - Peer Recovery Coach Charleston, SC
The Peer Recovery Coach reports to the Social Work Manager of Case Management and Care Transitions. Under practical guidance of a licensed Social Worker, the Peer Recovery Coach provides non-clinical services intended to aid patients in establishing recovery from high risk dependent alcohol and drug use problems. The Peer Recovery Coach will work as part of the Emergency Department team to provide peer support and motivation to encourage patients who are seeking treatment for alcohol or drug dependency and addiction issues. The Peer Recovery Coach will assist in completing referrals to treatment services and developing service plans to promote successful linkage to treatment services. Services will be provided in the Emergency Department with appropriate follow-up. This position is grant funded.
