The Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management. This position will have a primary caseload of patients in the Main Hospital.
MUHA GB64 - Social Worker (MSW) Charleston, SC
The Social Worker (MSW) reports to the Manager of Case Management. Under indirect supervision, the Social Worker (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. The Social Worker (MSW) completes an Assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and apply discharge interventions to lower the patient risk of an undesirable event post discharge. This position assists with the discharge process and promotes a successful transition out of the hospital through coordinated efforts between the health care team members to achieve targeted patient outcomes. The Social Worker bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
MUHA GB66 - Master's Social Worker Main Hospital
The Social Worker (MSW) reports to the Social Work Manager of Case Management and Care Transitions. Under clinical practice guidance of the Social Work Manager, the Social Worker completes a psychosocial assessment on the patient and caregivers, arranging outpatient follow-up as needed. Primary responsibility of this role is completing assessments of patients being cared for in the Emergency Department with mental health concerns. In collaboration with the health care team,the SW assists with coordination of the discharge process to ensure a successful transition to the appropriate level/type of care. The Minimum Experience and Training Requirements: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education required. Two years of work experience in the field is preferred. Majority of work will be in the Main Hospital Emergency Department.

MUHA EA32 - Registered Nurse II - Neonatal Nurseries Charleston, SC, Charleston, SC-Main Campus, Children's Hospital, Shawn Jenkins Children’s Hospital
The 66 bed Neonatal Intensive and Intermediate Care Units are located in our Children's Hospital. The MUSC NICU is the regional referral center for this area, providing care to premature and sick neonates. The average daily census is 66. The Neonatal ICUs provides medical and surgical care with most common diagnoses including prematurity, respiratory distress, and neonatal surgical conditions. In addition to respiratory and nutritional support, the NICUs provide ECMO/CRRT and surgical support. Pediatric Palliative care is available as needed. Two units located on the 8th floor of Children's Hospital include the 27 bed Neonatal Intensive Care Unit and the 9 bed Special Care Nursery. The 29 bed Level 2 Nursery is adjacent to Labor and Delivery on the 5th floor of MUSC's Main Hospital. The NICU also supports an infant home monitoring program for infants requiring monitoring upon discharge. All NICU beds are monitored beds. Under general supervision, the Registered Nurse II provides individualized, goal-directed patient care to families and patients at the competent level utilizing the principles and practices of the nursing process; delivers safe and effective care and interacts with other members of the health care team to achieve desired results.
