Under the direction of the Program Manager, the RN Specialty Coordinator delivers evidence-based, cost-effective and quality patient-centered care. Care coordination spans the entire healthcare continuum – from prevention, screening, diagnosis, treatment and rehabilitation to end of life. The goal of specialty care coordination is to reduce morbidity and mortality by eliminating barriers to timely access to care. The RN Specialty Coordinator must demonstrate proficient clinical knowledge to provide care coordination across continuum, disease-specific patient education, quality management, and patient-centered outcomes. The role utilizes the nursing process to promote decision making and timely access to quality patient care while providing a vital link in communication between multidisciplinary providers (internal and external). The RN Specialty Coordinator acts a primary point of contact for patients from initial referral through treatment, rehabilitation and end of life. Using the nursing model, the RN Specialty Coordinator directs care responsibilities for monitoring, coordinating care, educating patients and transitioning patients through the complex healthcare systems.
