The MUSC Care Transitions Coordination (CTC) Program Manager reports to the Director of Value Based Care Coordination for the MUSC Population Health Program, and works closely with the Population Health, Inpatient Case Management and Care Transitions, Primary Care and Post-Acute Care teams and leadership. The Program Manager supports departmental, ICCE, and Medical Center goals. Goals include achievement of clinical and quality measures, operational efficiency, and fiscal targets, with a focus on reducing hospital readmissions. The position is responsible for providing leadership, coordination and facilitation of all activities and services across the system regarding the care continuum. The CTC Program Manager will work in collaboration with inpatient and outpatient nursing, case management, social work, physicians, outpatient clinic offices, home health and other agencies that provide care across the continuum. The CTC Program Manager contributes to the improvement in the quality of life for patients, and the reduction of expenditures for the care of disease-related complications and/or social/environmental needs, by focusing on improving the coordinated management of patients at high risk for hospital readmissions and/or Emergency Department utilization. The Department of Psychiatry & Behavioral Sciences is seeking a Human Services Coordinator II to provide evidence-based treatment for adolescents with co-occurring post-traumatic stress disorder symptoms and substance use problems, primarily Risk Reduction Family Therapy (RRFT). The candidate will provide individual and family therapy sessions, which may be delivered in person, in the community (e.g., school or home setting), and/or via telehealth, as determined by safety and needs of the patient and family. The candidate will engage in phone calls and texts to patients and caregivers in between sessions as part of the treatment implementation and will coordinate with treatment providers of family members as well. The candidate will participate in clinical supervision in order to deliver this high-quality evidence-based treatment for adolescents with co-occurring posttraumatic stress disorder symptoms and substance use problems with fidelity. The candidate will also assist with implementation of SBIRT (Screening, Brief Intervention, and Referral to Treatment) among adolescents and family members presenting for treatment at the National Crime Victims Research and Treatment Center and related clinics. The candidate will engage in activities required to monitor treatment fidelity and retention and provide information as needed by the funding agency (SAMHSA). The candidate will engage in project meetings and community meetings and may make presentations to community agencies as it relates to adolescent substance use, traumatic stress, and impact on family.
