Position Summary
Job Summary:
The Assistant Social Worker at the MSU Center for Bleeding and Clotting Disorders plays a critical role in providing psychosocial assessment, care coordination, and advocacy for individuals and families affected by bleeding disorders, including hemophilia, von Willebrand disease, and rare clotting conditions. This role supports a multidisciplinary team including physicians, nurses, physical therapists, and other allied health professionals to deliver comprehensive, patient-centered care.
Job Duties:
- Conduct psychosocial assessments and develop individualized care plans for patients and families coping with chronic bleeding disorders.
- Provide emotional support, crisis intervention, counseling, and education to patients and families regarding diagnosis, treatment options, and psychosocial challenges.
- Coordinate community resources, benefits navigation, transportation, housing, financial assistance, and insurance support to ensure continuity of care.
- Collaborate with the multidisciplinary HTC team during comprehensive care visits, case conferences, and treatment planning meetings.
- Serve as an advocate for patients and families in clinical, community , and policy settings. Support patients with transition planning (e.g., pediatric to adult care), adherence issues, or challenges related to school, work, and social integration.
- Maintain accurate documentation in compliance with HIPAA, hospital policy, and grant reporting requirements (e.g., HRSA/CDC grants).
- Participate in data collection, quality improvement initiatives, and program evaluations as required by federal hemophilia program guidelines.
- Facilitate supp ort groups, workshops, or educational sessions for patients, caregivers, and community partners.
- Stay current with best practices in social work, bleeding disorder care, and relevant public health initiatives.
- Coordinates Children's Special Health Care enrollment and billing.
- Other duties as assigned.
Minimum Requirements
Knowledge equivalent to that which normally would be acquired by completing a bachelor’s degree in social work or a related human services field; up to six months of related and progressively more responsible or expansive work experience in counseling in a health/mental health environment; or an equivalent combination of education and experience.
Desired Qualifications
- Experience working in hematology, rare diseases, or with individuals with chronic health conditions.
- Familiarity with the structure and purpose of federally designated Hemophilia Treatment Center s.
- Bilingual (Spanish or other language), preferred but not required.
