Title:            Temporary Full Time, Community Health Worker - Rural Health Transformation Project - Grant

Location:      Stratford, NJ with travel to Cumberland County

Department: Academic Affairs & Student Services

Reports To:   Vice Dean of Academic Affairs & Student Services 

Summary:

Serve as a bridge between rural residents of Cumberland County and clinical health care services. Identify community health needs, engage patients through mobile outreach, conduct chronic disease assessments, and provide chronic disease health and wellness education.

Primary Duties/Responsibilities:

  • Work with the Project Director/Co-director to support mobile health unit operations and outreach, including scheduling and deployment in rural communities of Cumberland County.

  • Conduct outreach to engage residents of Cumberland County with or at risk for chronic health conditions.

  • Provide health screenings for blood pressure, diabetes, and other chronic health conditions.

  • Participate in specialized events for veterans.

  • Provide education on nutrition, physical activity, chronic disease self-management, disease prevention, and overall wellness during mobile outreach.

  • Facilitate patient navigation and care coordination to SUD treatment, health care clinics, preventive screenings, and mental health services. Schedule appointments, arrange transportation, and assist with referrals to insurance enrollment.

  • Screen for social determinants of health and provide referrals to food pantries, social services, housing, utilities assistance, legal services, and other support.

  • Maintain accurate records and documentation of outreach contacts, appointments, and services offered using electronic records.

  • Participate in data collection for evaluation of the project.

  • Assist in maintaining supplies on the mobile health unit and developing a welcoming environment on the unit for Cumberland County residents.

  • Provide nutritional assessments and assist in the implementation of innovative nutritional interventions to support healthy eating.

  • Work with members of the Rural Health Service Corps of medical students who will assist in screening residents for chronic diseases. 

  • Participate in all training required  for the grant.

  • Serve as driver of mobile unit

  • Attend meetings as required.

  • Understand and adhere to Rowan-Virtua SOM compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy

  • Keep abreast of all federal, state, and Rowan University regulations, laws, and policies as they presently exist and as they change or are modified.

Requirements:

This successful candidate will work on a mobile unit in Cumberland County, New Jersey. The hours will be 37.5 per week (full-time).  This role is a 6-month position with renewal possible. Must have a valid Driver’s License. 

Qualifications:

Completed training as a community health worker or in a related discipline.   Previous experience in community outreach, health education, and peer support is highly desired.  Veterans are encouraged to apply.  

SALARY INFORMATION 
  • In compliance with The State of New Jersey's Pay Transparency Act, the annual base salary range for this position is $55,000-$61,000, with placement within the range determined by candidate's qualifications, professional experience, internal equity, and other factors.
