Participates in post-conference sessions, family conferences, and follow-up procedures; prepares all written documentation; tracks evaluation process; performs liaison activities with families, involved agencies and professionals; and writes team summaries. Plans and implements follow-up programs with IDT. Provides short-term consultation services as time permits. Participates in the delivery of professional training activities. |
