| In this position, the Social Worker will provide comprehensive psycho-social assessment, management, and support to at risk patients and families in the Family Medicine and Internal Medicine outpatient clinics. In addition, the social worker addresses social determinants of health and complex issues, assisting and collaborating to make referrals with multiple community agencies to meet the needs of the patients and families. This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities. |
