Making Care Primary Program Manager	Syracuse
The Making Care Primary (MCP) Program Manager will report up through the Director of Quality for Upstate University Medical Associates at Syracuse Inc. and is integrated into the Primary Care Clinics. The MCP Program Manager performs processes necessary for ensuring the organizations success in the CMS Making Care Primary payment model.
Per Diem Specialty Pharmacist	Syracuse
As a Specialty Pharmacy pharmacist, this individual performs patient outreach assessments for determination of appropriateness of specialty medication therapy, and documents the care provided within the Specialty Pharmacy Patient Management Program software. This relates to clinical activities, metric reporting, compliance, and quality improvement projects for specialty pharmacy accreditation. This pharmacist will oversee program expansion, focus on developing relationships with clinic staff and specialty providers, help build third party payer and manufacturer relations and participates in all areas of dispensing, call center and clinic as needed or requested.
Nurse Practitioner/Physician Assistant-Connect Care Clinic	Syracuse
The Advanced Practice Provider will serve Full Time to provide care to patients within the Connect Care Clinic at SUNY Upstate’s Nappi Building at the Downtown Campus. The APP will provide management of patients recently discharged from the ED and hospital, as well as acute and primary care to patients in the outpatient setting.
Nurse Practitioner/Physician Assistant-Proceduralist	Syracuse
The fulltime Proceduralist Advance Practice Provider will provide care to patients within both the Inpatient and Outpatient areas at SUNY Upstate Downtown and Community Campuses.
Nurse Practitioner/Physician Assistant-Palliative Care	Syracuse
The Palliative Care Advanced Practice Provider will see patients with any chronic disease who may need additional support or advanced decision making. They will focus not only on providing comfort in dying, but also on living with the focus on controlling symptoms, supporting emotional, spiritual, and cultural needs and maximizing functional status. This position will see patients at both our Downtown Campus and Community Campus.
