We serve children ages 5-17 diagnosed with a variety of psychiatric and mental health disorders. Our multidisciplinary team utilizes Dialectical Behavioral Therapy in both individualized and group settings. | The Clinical Case Manager's primary responsibility is to facilitate patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, utilization of services and financial goals. Oversees the discharge process for all patients within the unit/core service area. The Case Manager is integral to the team effort to establish a safe and appropriate discharge plan/plan of care. It is essential that the case manager possess a working knowledge of the spectrum of hospital and post discharge services that are available. |
