Fixed Term 24 months - The purpose of the role is to ensure the best outcomes for our participants/workers by navigating and facilitating the access, management and cohesion of services and supports following discharge from hospital and during their time of adjusting and settling into community life following serious injury. It will involve establishing, coordinating and monitoring multi-disciplinary community-based rehabilitation programs with a focus on maximizing the participants/workers’ recovery and independence.
