Discharge Planner Coordinator

Permanent Part Time (24 hours per fortnight) 

About the Role

As a Discharge Planner, you will be working in a key role that provides clinical leadership to nursing, medical and allied health providers. You will be responsible for providing an effective discharge planning service, coordinating the strategic and operational planning for discharge activities and associated services across the hospital.

Duties and responsibilities involve but are not limited to:

  • Plan the patient discharge process and complex discharges with a multidisciplinary team
  • Ensuring patients have a smooth transition from the hospital to the community with appropriate care and services
  • Monitoring patient Length of Stay
  • Developing relationships with external support services/networks (e.g. residential facilities, TCP and community care services)
  • Completion of initial needs assessment and commencement of time-critical patient care plans with a goal to optimise the patient's health and wellbeing
  • Providing patients and their families with assistance and support in relation to their injury or illness

About You

The successful candidate will meet the following criteria:

  • Registration with AHPRA as a Registered Nurse
  • Minimum of three years’ post registration experience with experience in acute care
  • Knowledge of and ability to implement contemporary discharge planning practices
  • Ability to plan and conduct staff education
  • Demonstrated effective interpersonal and communication skills (verbal and written)
  • Advanced level of problem solving, prioritisation and negotiation skills
  • Ability to work without direct supervision in a multidisciplinary, fast-paced team environment
  • Knowledge of and participation in Quality Improvement process
  • Sound computer skills

Desirable criteria:

  • Previous DVA experience
  • Previous experience as a Discharge Coordinator

Remuneration

On offer is a full-time equivalent, base annual salary ranging from; $70,051.89 - $92,182.36 + superannuation based on experience level and qualifications, exclusive of penalties or allowances. (WA gr1-gr8)
