We are seeking an Orthopaedic Care Navigator to join our team on a part time basis

 

About the Role

The care navigator role would be suitable for Nursing and Allied Health professionals. The role will be responsible for applying advanced knowledge and skills to facilitate the continuity and quality of care for patients admitted for orthopaedic procedures.

  • The role will provide support and guidance to the patients from the point of referral to the hospital and for a period of 6 weeks post discharge.

 

The aim of this position is to ensure that the patient is supported as they undergo care, and they have access to the Ramsay Integrated Care Strategy.

  • The role will collaborate with other teams that are integral to the multidisciplinary care of the patients. these include but are not limited to preadmission and discharge teams, as well as nursing, allied health and the VMOs.
  • The role will be fundamental to ensuring a positive experience for the patient and will promote an end-to-end pathway across Ramsay services, in and out of the hospital setting.
  • The role will also act as a central resource for the hospital in promoting best practice in the provision of patient care through quality improvement initiatives and service innovation at site level, collaborating with other hospitals and corporate teams as needed.

 

About You:

As this is a specialist role, we are seeking a candidate with at least 7 years’ professional experience and demonstrate high levels of clinical and managerial experience across orthopaedic settings.

You will be responsible for

  • Ensure a high level of patient satisfaction
  • Supporting orthopaedic bed coordination
  • Strong commitment to quality care, continuous improvement
  • Excellent communication with all stakeholders (internal and external)

 

Essential Criteria

  • Registered with AHPRA as a Registered Nurse or Allied Health Professional
  • Minimum of three years post registration in speciality area of Orthopaedic practice or relevant experience in advanced practice
  • Understanding of scope of practice in line with relevant registration
  • Demonstrated experience in continuous improvement and initiatives supporting the customer experience
  • Understanding of health fund requirements for orthopaedic services (desirable)
  • Proven project management skills with sound organisational and change management skills
  • Computer literacy.

Requirements

  • Must provide a National Police check conducted within the previous 12 months
