REPORTS TO: Intake Manager
HOURS: Weekends, Holidays, and Observed Holidays
RELATIONSHIPS: Weekend Clinical Manager, patients, families, caregivers, physicians and office staff, community vendors, contract and regulatory community agencies, third party payors, UR Medicine Home Care (URMHC) departments and staff.
I. Care Management:
A. Completes referrals from emails and faxes for URMHC home care services under the supervision of a Registered Nurse (RN) referencing them for assistance with complex situations.
B. Evaluates patients through chart review and phone interview for appropriateness for appropriateness for home care including evaluation of the safety, support system, identified caregivers to learn/perform care, patient willingness/ability to perform care, and
C. Possesses and utilizes knowledge of agency and community resources to make appropriate referrals for services, supplies, equipment, and arrange for timely delivery of such.
D. Identifies the home care focus of care.
E. Involves patient and family support system in the referral process to assure the maximum involvement in patient care needs at home.
F. Communicates and coordinates referral process with the interdisciplinary team at UR Medicine Home Care and at institutions as well as with patients including documentation per institution and UR Medicine Home Care standards.
G. Actively participates in marketing UR Medicine Home Care services to referral sources ongoing.
H. Assists weekend Communication and Triage Specialist with patient call management.
I. Performs other duties as assigned.
II. Financial Management:
A. Plans referrals with attention to reimbursement issues and efficient use of resources as well as taking responsibility for own productivity.
B. Initiates assistance of UR Medicine Home Care Patient Financial Services for patients without insurance.
C. Utilizes UR Medicine Home Care consultants to assure maximum efficiency in care delivery through use of new/improved technology and products.
D. Communicates with patient and family the need to identify able and willing caregivers to be involved in learning and providing care to maximize efficiency in care delivery.
E. Maintains productivity in answering calls and generating referrals as well as assisting others as needed to limit use of overtime and supplemental help.
F. Participates in marketing and identification of new opportunities to grow referral volumes.
A. Maintains attendance within UR Medicine Home Care standards.
B. Complies with agency and regulatory standards.
C. Completes required/mandatory in-services, professional licensing requirements and employer and institutional requirements.
D. Answers phone calls within 3 rings.
E. Reads emails frequently throughout every work day.
F. Adheres to standards for documentation of referrals.
G. Complies with Corporate Compliance standards including confidentiality and HIPAA requirements.
H. Complies with safe work practices (ergonomics, OSHA).
I. Referrals are timely, accurate, complete and communicated to Care Team Coordination.
J. Shares in responsibility for team maintenance of logs, statistics and other duties assigned.

C. Attends team meetings and in-services.
D. Participates in mentoring/training new staff.
E. Attends educational opportunities and shares knowledge with others.
F. Works with team to assure maximal efficiency of resources.
G. Takes initiative in problem solving issues.
H. Responsible for the Key Performance Indicators (KPI’s) established by their manager.
1. Current active NY State LPN License in good standing required.
2. One year of Home Care in a CHHA or related experience preferred.
3. Strong written and verbal communication skills for interactions with patients/caregivers/referral sources/staff.
4. Proficient with phone and customer service skills.
5. Computer competency required. Knowledge of Excel and Microsoft Outlook preferred
