We are seeking a compassionate and skilled health professional to join our Statewide Progressive Neurological Disease Service (SPNDS) as a Clinical Care Liaison, primarily supporting people living with Motor Neurone Disease (MND). This meaningful role walks alongside clients and their families from diagnosis through every stage of the disease, including end‑of‑life care. You will coordinate care, provide clinical guidance, and work closely with Neurologists, allied health teams, and community providers to ensure timely, person‑centred support.
