Public Health Nurse Sr in Chesapeake, Virginia
Chesapeake Health Department, (CHD), is seeking compassionate and experienced Registered Nurse to serve as a Senior Public Health Nurse (PHN, Sr.) in our maternal & child health (MCH) and Baby Care program area. This position provides case management services, in a home-based setting, for medically and socially underserved maternity and pediatric clients in the city of Chesapeake. The Baby Care program plays a vital role in promoting healthy pregnancies, positive birth outcomes, and early childhood development in vulnerable populations. The PHN, Sr. may supervise and mentor ancillary members of the MCH team. Responsibilities include but are not limited to monitoring program performance and client outcomes, data analysis and quality improvement to assess program effectiveness, identifying areas for improvement, and working collaboratively with the interdisciplinary team to ensure adherence to operational best practices and program guidelines.
