About the Role
As a Care Coordinator, you will oversee and improve the delivery of our Aged Care services in Rockhampton. Your responsibilities will include managing referrals, supporting clients as they begin receiving our services, and regularly updating care plans to reflect their changing needs. You will arrange home visits to ensure the people we support receive the care they require, while building collaborative relationships with clients and their families.
You will also play a key role in program coordination by evaluating, refining, and managing operational and administrative systems. Your focus will be on providing outstanding service that ensures clients and their families feel supported, while contributing to the ongoing growth of our Aged Care services.
This is a Permanent Full-Time position based in Rockhampton, with occasional travel to clients' homes throughout the region as required. This presents an exciting opportunity for the right candidate to take full ownership of the role and advance their career.
Key Responsibilities
- Conduct client assessments and develop care plans with clients, representatives, and stakeholders
- Update care plans regularly and conduct home visits
- Monitor clinical risks and client needs, ensuring appropriate referral pathways are in place
- Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs
- Develop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care clinical case management experience
- Understanding of Aged Care funding and service delivery, specifically Support at Home
- Confidence in utilising a variety of technology including mobile devices and computers
- Proficiency with Microsoft Office, Outlook and database systems
- Ability to effectively build relationships with diverse stakeholders
- Strong communication skills
- Ability to multi-task and work autonomously in a fast-paced environment
- A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
