Future Pathways Lead Practitioner

Future Pathways is part of AV’s approach to the Department of Families, Fairness & Housing’s (DFFH) state-wide initiative to support Family Preservation & Reunification (FPR). The Future Pathways team compliment a suite of family supports provided within the FPR model. 

The Future Pathways program is an innovative approach built on practice elements such as motivational interviewing, communication skills building and goal setting to respond to identified needs and achieve real and demonstrable outcomes. Working collaboratively, the team seek to address key intersectionality issues with families, children and young people (e.g. substance use, mental illness, low income, social isolation) as identified by each regional FPR Response. 

This Role 

The Future Pathways Lead Practitioner will:  

• Lead a team in supervision, reflective practice and Case Consultation meetings. 

• Oversee and coordinate referrals ensuring the Future Pathways team work collaboratively with Child Protection Practitioners, services and families to ensure risk assessments and safety plans are maintained and tracked 

• Ensure the Future Pathways Practitioners provide a targeted intervention in the family home at flexible times and when they are most receptive to learning; this may include early mornings, evenings. 

• Fulfil program requirements regarding case recording, data collection, registration standards and other requirements, such as FPR evaluations. 

• Monitor program performance and outcomes through the collection and analysis of service data and implement changes to the operation of the program. 

• To work in conjunction with the Program Manager and EBM Practice & Implementation. Lead to plan and implement responses in relation to program, sector and service developments. 

• Identify performance trends, best practices, and opportunities for improvement. 

• Contributes to the performance monitoring and reporting requirements of the Future Pathways program. 

• Ensure the Future Pathways Practitioners strengthen family connections, promoting placement prevention and the safety of children, young people and parents. 

Role Requirements  

To be successful in this position you will need:  

• A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences at degree level with relevant experience; or diploma level with substantial experience. (Refer to Position Description for overseas qualifications) 

• The ability to demonstrate competency against the knowledge, skills and personal qualities outlined on page 2 of the Position Description – such as: 

• supervising multi-disciplinary staff providing coaching and feedback  

• leading reflective practice discussions and team meetings 

• working with high needs complex families  

• using a child focused family-centred approach 

• developing intervention goals and plans relating to the risk of out-of-home placement 

• utilising culturally sensitive practice with marginalised and minority groups 

• knowledge of child welfare services 

• understanding theoretical approaches (i.e., motivational interviewing and social inclusion) 

• knowledge of evidence-based interventions 

• experience conducting risk and needs assessments 

• working within a casework framework 

• The ability to perform work outside of standard office hours to service the needs of clients 

• A current Victorian driver’s license and Working with Children Check and be prepared to undergo Criminal History Check(s) prior to commencement
