The primary goal of the position is to provide phone based, face to face and outreach intake and active hold support across Western Melbourne for school disengaged young people 10-17yrs and their parents/carers whilst awaiting long term case management assistance.

This 0.8 FTE position, fixed term to 31st December 2026. Support up to 40 clients/families who are awaiting case management support.

 

The Program:

The Navigator Program is an intensive, targeted, education support service Navigator staff work in collaboration with schools, young people, families, and external stakeholders to find ways to achieve educational goals.

Many of these young people have complex barriers to educational engagement & have not responded to school-based engagement interventions. Working with young people, their families & support networks to address issues underlying disengagement & help them re-engage with their education & achieve their goals.

 

About the role:

We are currently looking for experienced Intake and Active Hold Worker for our Navigator program in Western Melbourne

You will be supporting referrals for young people (10 - 17yrs) who have disconnected from school and assisting their carers/guardians. You will work collaboratively within a multi-disciplinary team, comprising of a Team Leader, Senior Case Managers, Brief interventions Worker, a Groups Facilitator and other relevant services, bringing expertise and valuable understanding in speciality aspects pertaining to current disengagement challenges.

 

The skills you will use day to day:

  1. Complete intake assessments which gather client information and begin to explore their strengths, interests and barriers to mainstream education
  2. Alongside the Team Leader, present Active Hold cases at the Navigator Allocations Meetings
  3. Develop and maintain a professional and respectful working relationship with young people, their families, schools and other referrers
  4. Provide appropriate referral information for other support services available.
  5. Maintain intake information and assessments as required for the delivery of the Navigator Service.
  6. Regularly record information on the department’s data management system
  7. Work as part of a team and participate in team meetings, joint work opportunities and case evaluation activities.

 

 What you need to know:

  • A salary at SCHADS level 5
  • Flexible through the increments up to 5.3 (depending on experience)
  • Contracted position until 31st December 2026 (with the possibility of extension)
  • Site based at our office in Yarraville
  • Pool cars available for your use.
