A bit about the role
We are seeking a compassionate and highly organised Hospital Liaison Discharge Officer to join our dedicated MCH team and help ensure a smooth transition for families from hospital to community-based care.
You will act as the key link between maternity hospitals and the local MCH service.
What you’ll deliver
- Coordinate timely and accurate referrals from hospitals to the MCH service.
- Facilitate early engagement of families with MCH services post-discharge.
- Liaise with hospital staff, MCH nurses, and other stakeholders to ensure continuity of care.
- Maintain accurate records and data entry in line with privacy and health information standards
About you
You are a proactive and empathetic professional with:
- Experience in a health or community setting (preferably maternal and child health or hospital discharge planning).
- Excellent communication and interpersonal skills.
- Strong organisational and administrative abilities.
- Knowledge of relevant legislation, privacy, and child safety standards.
- A qualification in health, nursing, midwifery or a related field is desirable.
- A positive, collaborative approach to delivering best practise Maternal and Child Health services
- A drive to learn and support families with the latest information that supports best outcomes
- Current AHPRA Registration as a Division 1 Nurse and Midwife
- Willingness to undergo pre-employment background checks including Criminal History, Working with Children, Victorian Driver’s License, Functional Assessment
- A curious, collaborative, and inclusive community mindset
