Systems Coordinator
Full-time | Permanent
Based in Parramatta CBD | Hybrid model (3 days in office, 2 days from home)

Are you passionate about quality care in the Home and Community Care (HACC) sector? We’re seeking a dedicated Systems Coordinator to lead and optimise the use of our client management system, ensuring it’s used effectively across our HACC teams to drive quality outcomes for those we serve. In this role, you’ll partner with local HACC and RAC teams, business support, and Seniors Services leaders acting as a subject matter expert to help us reach the highest standards of client service.

Key Responsibilities:

  • Act as the primary contact for Home & Community Care teams, supporting the use of the client management system.
  • Develop, monitor, and uphold system policies, procedures, and instructions.
  • Promote and support consistent adoption of the client management system across HACC, providing expert guidance on its implementation and maintenance.
  • Regularly measure client outcomes in line with our model of care, supporting high-quality service delivery.
  • Proactively manage system, data, and non-compliance issues.
  • Conduct reviews and audits to ensure systems access, best practices, and optimisation.
  • Analyse program outcomes, identifying trends to guide improvements and practice reviews.
  • Provide ongoing support to build quality assurance and capability in system use among HACC teams.
  • Offer content expertise as a Subject Matter Expert to both HACC teams and business support.
  • Drive continuous improvement across the Home and Community Care and Business Support streams.
  • Coordinate projects related to system and quality management initiatives.
  • Engage with other Uniting teams to streamline processes, eliminate duplication, and foster continuous improvement.
  • Oversee change control and version upgrade activities, ensuring smooth communication and collaboration with stakeholders.
  • Participate in quality management systems and processes.

Qualifications and Experience:

  • New graduate looking for an opportunity to gain experience in Systems Coordination/Or a minimum of three (3) years’ experience in a similar role.
  • Strong written and verbal communication skills with a systematic, organised, and thorough approach.
  • Ability to navigate complex organisational structures and manage influence-based relationships.
  • Deep understanding of the Aged Care industry, with particular expertise in home & community care.
  • Strong Excel skills and good knowledge and capability with Data Analytics
  • In-depth knowledge of client management systems.
  • A demonstrated passion for social change and commitment to supporting the most disadvantaged.

Desirable:

  • Postgraduate qualifications in a related field.
  • Knowledge and experience with AlayaCare, Clinical Manager or other Client Management Systems
