DES, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as an Incident Reporting Coordinator. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

Job Duties:

Essential Duties and Responsibilities include but are not limited to:
• Entering incident report into the Incident Management System (IMS), Arizona Health Care Cost Containment System (AHCCCS) Portal, notifying key personnel (including Support Coordination, Quality Assurance and Region Administration) of these incidents, and tracking of this communication and following up
• Researching each incident report through the Client Application database and the District Employee Directory in order to ensure member, vendor and internal distribution information is correct; compiling reports and sending via interoffice mail to each Support Coordinator
• Answering the incident reporting phone line and documenting the calls via log, ensuring entry into IMS; following up with the afterhours emergency staff and the afterhours administrator to ensure information is accurate and thorough for Quality Assurance review and possible entry into IMS; following up with vendors for written reports on incidents that were verbal notifications
• Conducting training and technical assistance for qualified vendors/service providers and division staff

Knowledge, Skills & Abilities (KSAs):

Knowledge of:
• Human dynamics and organizational systems
• Routine letter and memo composition
• Investigation and fact finding methods
Skill in:
• Oral and written communication
• Compiling, organizing, analyzing, and interpreting data and information into report formats
• Working with material of a technical and/or confidential nature
• Dealing with the public using appropriate tact and resourcefulness
Ability to:
• Prioritize and meet timelines
• Screen and route correspondence and request follow up
• Use various computer software applications (Word, Excel, Database systems, Google Docs, etc.)
• Assist with timely projects, reports, memos, etc

Selective Preference(s):

The ideal candidate for this position will have:
• One year of experience working with individuals with developmental disabilities or a vulnerable population; experience utilizing various computer programs (e.g. Word, Excel, Database systems, Google Docs, etc.) is highly desirable

Pre-Employment Requirements:

• Arizona Level 1 Clearance Card is required
• Candidates for this position shall be subject to a search of the Child Protective Services Central Registry pursuant to A.R.S. § 8-804
• Successfully pass background and reference checks; employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions

Retirement:

State employees are required to participate in the Arizona State Retirement System (ASRS), the State sponsored retirement contribution plan and the Long-Term Disability (LTD) program after a 27-week waiting period. The ASRS defined benefit plan provides for life-long income upon retirement. You will also have the option to participate in a voluntary deferred compensation program to take advantage of tax-deferred retirement investments.
