The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Habilitation Services Project Coordinator. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high-quality, and member-driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.
Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
Essential Duties and Responsibilities include but are not limited to:
- Coordination of investigations, conducting investigations, and completing investigation reports. Compile reports and send via interoffice mail to each support coordinator.
- Entry of incident report into the Incident Management Systems (IMS), notification of key personnel (including Support Coordination, Quality Assurance and Region Administration) of these incidents, and the tracking of this communication and follow up. Research each incident report through the Client Application database and the District Employee Directory in order to ensure member, vendor and internal distribution information is correct.
- Conducts training and technical assistance for qualified vendors/service providers and division staff.
- Drives to and from licensed settings to complete health and safety reviews.
- Answers the incident reporting phone line and documents the calls via a call log, ensures entry into IMS. Follows up with the after hours emergency staff and the after hours administrator to ensure information is accurate and thorough for Quality Assurance review and possible entry into IMS. Follows up with vendors for written reports on incidents that were verbal notifications.
- Receives incident reports via fax, email and verbal notification. Performs initial review for seriousness and priority before compiling for Quality Assurance Review.
The ideal candidate for this position will have:
- At least 1 year of experience providing services to individuals with developmental disabilities.
- Experience conducting investigations is highly desirable.
- Experience utilizing various computer programs (e.g., Word, Excel, Database systems, etc.) preferred.
