    The Adult Empowerment Services are committed to improving the quality of life for persons with disabilities through person-centred services that empower individuals, support their families, and foster inclusive communities. Comprising the Day Activity Centre, Disability Case Management Programme (DCMP), and Young Adults Activities!, these programmes aim to help individuals reach their fullest potential, achieve greater independence, and lead meaningful, fulfilling lives.

    The Disability Case Management Programme (DCMP) provides holistic and timely case management and interventions to address the complex needs of persons with disabilities, ensuring comprehensive care and support. DCMP clients often have multifaceted support needs that require collaboration across multiple agencies and professionals.

    As part of the Interdisciplinary Care Planning (ICP) team in DCMP, the Life Coach will work collaboratively with clients, their families, and other professionals to implement care plans, coordinate care arrangements, and report progress to the ICP team. The goal of the role is to empower clients, enhance their independence, and support their integration into inclusive communities.

    Critical Work Functions

    Implement Care Plan  

    • Work with clients to implement individualized care plans that address their personal goals, aspirations, and support needs.
    • Support clients to develop life skills, build confidence, and work toward greater independence in their daily lives.
    • Facilitate targeted activities and interventions that promote personal growth, social integration, and overall well-being.

    Facilitate Holistic Client Development

    • Build positive relationships with clients, by providing a safe physical and emotional environment for their continual development 
    • Facilitate and manage clients’ skills building proactively through modification and accommodation of resources and environment, to enhance motivation and opportunities for learning
    • Provide opportunities for skills building within the set environment as well as externally to extend the learning into the community

    Coordinate Care

    • Coordinate care arrangements by collaborating with families, caregivers, and professionals from various agencies to ensure seamless service delivery.
    • Serve as a point of contact, where assigned, to ensure clients receive timely and appropriate support across healthcare and social service domains.
    • Identify gaps or challenges in care arrangements and escalate them to the Interdisciplinary Care Planning (ICP) team for resolution.

    Report and Document Progress

    • Monitor and evaluate client progress, providing regular updates and insights to the Interdisciplinary Care Planning (ICP) team.
    • Maintain and monitor client records related to service utilisation in accordance to organisational and programme requirements
    • Participate in the development, monitoring, and evaluation of outcome measures to assess programme effectiveness and impact.

    Job Requirements
