The Social Work Associate supports the social worker in providing psychosocial support to the students and their families. The Social Work Associate will also bring to each case a basic understanding of the biopsychosocial concerns of families and the influences of family, community and cultural differences, enhancing the case management by the interdisciplinary team. The postholder may work across a variety of settings at Rainbow Centre (early intervention, schools, out-of-school hours care and community outreach services etc).
Key tasks and responsibilities:
Casework
Individual and Family
- Identify the client’s and family’s needs and ensure that they receive appropriate services.
- Conduct basic psychosocial and risk assessment for cases by taking into account the integration of a range of information independently
- Collaborate with clients and other key service stakeholders (teachers, therapists, service leaders, other agencies etc.) in developing and implementing an effective intervention plan.
- Conduct home visits and accompany clients for appointments (i.e medical consultations, court hearing etc) independently and provide assessment of the home situations or organisations to Social Worker.
- Produce brief social reports and documentations (include case notes) for the social worker for continuation of follow up.

Service Administration & Management
- Data Management: Maintain and monitor client records related to service utilization in accordance to organizational and professional requirements.
- Resource Management: Maintain inventories of equipment, materials and supplies.
- Health & Safety: Ensure that the health and safety policy is adhered to and all equipment are properly and safely stored and maintained for safe use.
Continuing Professional Development
- Remain up to date and compliant with all relevant legislation, organizational procedures, policies in order to uphold standards of best practice
Job Holder Requirements
- Diploma holders. Preferably in Social Work/Community Service/Human Services
- Experience working in a disability/social work setting will be an advantage
