Job Description:
The Clinical Pharmacist is responsible for cardiology-based chronic disease management and anticoagulation management. This includes initiating, modifying, discontinuing, and monitoring warfarin and other anticoagulants within their scope of practice, while also managing conditions such as heart failure, hypertension, dyslipidemia, and anti-arrhythmic monitoring. Responsibilities encompass patient education, medication education, and the development and monitoring of therapeutic plans for both cardiology and anticoagulation patients. This pharmacist collaborates with the healthcare team to optimize pharmacotherapy and provides training to staff, pharmacy residents, and students in both areas. Performs other duties as required.
Minimum Qualifications:
1. Must hold current and unrestricted Ohio Pharmacist License
2. Doctor of Pharmacy degree required or 5 years clinical experience in the area of cardiology
3. Residency training preferred
4. Preferably meets ASHP residency preceptor required criteria
5. Record of scholarship and previous experience in clinical ambulatory setting
6. Board certification preferred
7. Ability to communicate and interact effectively, professionally and courteously with other health care professionals, patients and members of the University community
8. Demonstrates effective and culturally appropriate communications
9. Utilizes effective communication to provide excellent customer services and a positive public image
10. Must be able to withstand pressure due to work volume, interruptions, pace, job variety with an expected degree of accuracy
