About the role

 
This role is based in Wagga Wagga and is a full time role working Monday to Friday. The Women's Wellness and Recovery program is a community based program that supports women that are pregnant or have young children and require support with substance use issues. The AOD Case Manager works to reduce alcohol and other drug related harms by providing evidence-based drug and alcohol services. This role does require travel throughout the Murrumbidgee Area Health. Travel is shared across the team and will be limited to set days.  
r main duties will include:
  • Risk Assessment
  • Care planning and coordination of services
  • Education, brief intervention and harm minimisation

 

ABOUT YOU

  • Relevant qualifications that support case management and care coordination including, Social Work, Nursing Degree, Diploma in Community Services
  • Knowledge and/or experience in delivery of evidence based brief interventions for drug and alcohol problems including harm reduction, motivational interviewing and relapse prevention
  • Current Drivers License, certificate
