About the role
As the Clinical Care Coordinator you will coordinate and lead the care team in consultation with the Facility Manager. You will ensure compliance with legislative and statutory requirements to enable us to continue to deliver a high standard of care and most importantly, a quality lifestyle for our residents.
Working within a supportive and dynamic team, you will be provided with a challenging and diverse work environment, as well as an opportunity for you to contribute and make a real difference to the residential aged care community.
We currently have a full time permanent opportunity for an experienced Clinical Care Coordinator for our facility in Eleebana, to work Monday to Friday.
St Francis is home to 52 residents, and also has a 12-bed Dementia Specific Secure Unit.
About you
- Experience in aged care or closely related medical/PEOL care along with dementia experience

Strong background in clinical and people leadership and the ability to support, lead and mentor a multi-disciplinary team

Registered Nurse with current AHPRA registration and a commitment to contemporary nursing practice

Knowledge and understanding of the legislation governing the Aged Care

Experience in assessment and the Aged Care Funding Instrument (ACFI)
