The Role:
Our Care Consultants have experience with Case Management / Care Plans. You'll be visiting our clients in their homes: located in the North-West suburbs of Metro Melbourne, to do the following:

• Initial Customer assessments & service planning
• Work with your client to understand what they want from their Home Care Package, and then help them to build it based on their level of funding
• Care planning and regular monitoring and reassessment of customer needs
• Providing ongoing case management to HCP and or private customers to meet service and care plan goals

For You:
You'll be in the field 70-80% of the day, visiting clients. You'll have the option to work from home in between visits for report writing and case notes.

Key Selection Criteria:

• Qualification in Case Management, or
• Undergrad qualification in Nursing / Allied Health / Social Work is essential
• Div 1 Nursing Qualification - desirable but not essential
• Prior case management experience will be highly regarded
• Experience completing initial and annual customer assessments and care planning
• Knowledge of the Home Care Package industry
• Must be willing to have a flu vaccine and undertake a police check
• Must have Covid-19 Vaccinations
