About the role:

The Assessment Officer role at Baptcare primarily involves conducting initial assessments and annual reviews for in-home care customers. The main responsibilities include:

  • Completing comprehensive assessments for new customers to determine service eligibility and required care plans
  • Performing in-home visits following protocols to assess customers respectfully and timely
  • Managing entire customer onboarding process from referrals to service scheduling
  • Thoroughly documenting assessments per guidelines across relevant systems
  • Liaising with customers to clearly explain available services
  • Handling administrative tasks and escalations related to customer assessments
  • Ensuring accurate and up-to-date customer documentation

 

The role requires excellent assessment skills, professionalism in home visits, strong documentation abilities, and effective communication with customers about care services. The Assessment Officer is crucial in determining appropriate care plans and facilitating seamless service delivery for new in-home care customers.

About You

As an experienced professional, you possess a proven capability to perform assessments accurately, both during home visits and over the phone. Your attention to detail and efficiency enable you to consistently meet set deadlines, ensuring a seamless experience for clients.

To thrive in this role, you should possess:

  • Minimum Certificate III in Aged Care or equivalent
  • Ability to use computer-based accounting and finance systems
  • Willingness to take responsibility and accountability for one's own work
  •  Ability to work without supervision
  •  Proficiency in using Microsoft Office Suite
  • A positive can-do attitude with good problem-solving skills
  • Strong written and verbal communication skills with the capacity to maintain the personal dignity and confidentiality of all customers and their representatives
