Targeted Care Packages Case Manager

About the Role
As a Targeted Care Case Manager, you'll work alongside children, young people, parents, carers, and families facing complex challenges to help keep children safely at home or support their transition from care. Using a trauma-informed, family-centred approach, you'll partner with Child Protection and a range of community services to deliver coordinated, wrap-around support guided by the Best Interests Case Practice Framework.
As part of BaptistCare's Community Services team, you'll manage a diverse caseload of families receiving Targeted Care Packages, working collaboratively to improve safety, strengthen family relationships, and achieve positive long-term outcomes for children and young people. This is a rewarding, community-based role that combines direct client engagement with collaborative case management.
Day to day, you will:
- Provide intensive case management to children, young people, parents, and carers receiving Family Options and Targeted Care Packages.
- Conduct family preservation and reunification assessments, identifying strengths, risks, and opportunities to support children to remain safely at home or transition successfully from care.
- Develop, implement, and regularly review trauma-informed case plans in partnership with families, Child Protection, and other support services.
- Build strong, trusting relationships through regular face-to-face engagement with children, young people, and their families.
- Coordinate and monitor service delivery across internal and external providers to ensure holistic, responsive support.
- Complete carer reviews, monitor placements where required, and provide case management for Child Protection-contracted clients.
- Maintain accurate case records and ensure compliance with funding, legislative, and BaptistCare quality and safeguarding requirements.
What You'll Bring
- A qualification in Social Work, Psychology, Community Services, or a related discipline
- Demonstrated experience managing complex, high-risk cases within community, aged care, or social services settings is highly desirable
- Applied understanding of trauma-informed practice and the ability to work confidently with clients experiencing significant disadvantage
- Solid knowledge of the Targeted Care Packages or Home Care funding framework
- Confidence working within BaptistCare's values of respect, justice, and human flourishing - especially under pressure
- Current Victorian driver's licence and readiness for regular community travel across your client area
