Eastern State Hospital, located in Williamsburg, Virginia, was founded in1773, with a well-intentional emphasis on community focused mental healthcare. This state of the art campus, situated on 500 acres, consists of two patient care buildings with 300 patients, and a highly skilled staff of over 900.Eastern State Hospital is part of the Department of Behavioral Health and Developmental Services (DBHDS) system, and is accredited by the Joint Commission.

Eastern State Hospital invites qualified applicants for the position of the Social Work Case Manager (Counselor I). This position is responsible for providing social work services and discharge planning for patients at ESH and serving as the link between community agencies and others that support the patient’s recovery and return to the community. The qualified candidate will have a comprehensive knowledge of Social Work theory and practice with individuals and systems, functions within hospital/community mental health systems and discharge planning.

Responsibilities include, but are not limited to:

  • Develop and implement person-centered treatment and discharge plans in collaboration with the patient/family/guardian/Authorized Representative and other members of the interdisciplinary treatment team.
  • Develop discharge goals in the individualized recovery plan and ensure patient understanding of their treatment, discharge criteria and discharge goals.
  • Serve as the link between community agencies and others that support the patient’s recovery and return to community.
  • Coordinate community and agency resources and build effective partnerships with community service providers and interdisciplinary treatment team.
  • Attend interdisciplinary treatment team meetings and assist in the development of comprehensive individualized recovery based treatment plans. Facilitate at least one therapeutic group per week.
  • Provide counseling and education regarding discharge issues, community resources and individualized recovery plans.
  • Under supervision and review of the team clinical social worker, provide psychosocial assessments as assigned and within designated timeframe.

Minimum Qualifications

  • Bachelor’s degree in Social Work is required.
  • Experience providing case management and discharge planning services to the mentally ill population.

Additional Considerations

  • Experience interviewing, assessing, and. problem solving and ability to coordinate community and agency resources and work effectively with others in the resolution of problems.
