Job Duties

Western State Hospital is seeking a full time Hospital Investigation Assistant who will be responsible for working in collaboration with the Assistant Risk/Patient Relations Manager, the Hospital Investigator, and other team members in assisting in ongoing compliance with applicable patient safety TJC accreditation standards, Commonwealth of Virginia legal and regulatory requirements, and the Center for Medicare and Medicaid (CMS) Conditions of Participation; assisting in the monitoring and coordinating of hospital-wide compliance with DBHDS risk management and investigation policies and procedures for the reporting of patient, employee and visitor incidents and complaints; and assuring confidential investigation of all specified patient incidents and their reporting to the Virginia Office of the Attorney General, Offices of the State Inspector General, DBHDS, Office of Human Rights, Adult Protection Services, and the disAbility Law Center of Virginia (dLCV).

The Hospital Investigation Assistant will also be responsible for initiating the investigative process to include abuse, neglect, exploitation, patient incidents, and alleged human rights complaints when applicable or as requested by the Facility Director/designated representative, Assistant Risk Manager, Risk Manager or the Hospital Investigator; maintaining risk and investigator related investigation information, statistical profiles, etc., in compliance with The Joint Commission, and other related state/federal standards; analyzing received complaints and identify exceptions, trends and level of seriousness of alleged human rights complaints; ensuring accuracy of reported information by interviewing staff, reviewing CCTV video, access control reports, electronic medical record entries, or other available sources of data.

Minimum Qualifications

- Knowledge of and experience with investigative techniques, organizational skills, critical thinking, writing, and interviewing skills
- Direct patient experience
- Ability to communicate effectively, both orally and in written form with a variety of individuals from a variety of professional backgrounds
- Experience working independently, which requires initiative, attention to detail, decision-making, and the ability to assess and plan a workday based on changing priorities
- Proficiency and experience with MS office products, such as Word, Excel, Power Point, and Outlook
- Ability to utilize CCTV hardware in investigations
- Ability and experience collecting and interpreting data or highly confidential information in a time effective manner with attention to detail
- Ability to complete mandated investigation training by DBHDS and be on an approved list of investigators
- Ability to complete mandated investigation training by DBHDS and be on an approved list of investigators
