The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 35 local health districts.

The Richmond & Henrico County Health Districts (RHHD) are on the forefront of Public Health and Population Health. Our mission is to protect health, champion equity, and partner to address local needs. We accomplish this by exposing and addressing the root causes of health disparities and protecting health through disease prevention and monitoring, providing clinical services, nutrition services, environmental health services, vital records, emergency planning, and building health equity by partnering with communities and working collaboratively across sectors. The Richmond City Health District is currently seeking a Population Health Community Coordinator. The Population Health Community Coordinator serves as the Healthy Communities Coordinator for the local health district’s efforts to work on developing and implementing environmental strategies to improve population health. Represents the health district at community events and assists, as necessary, in the coordination of health community activities. Supports and reinforces healthy behaviors in the district by working with community coalitions, schools, businesses, city leadership, urban designers and other local organizations and partners to build support for lifestyle change.


This incumbent will be expected to work in our Richmond City Health District and other offsite areas in the community as needed. Mileage reimbursement is available for any work-related travel. Typical work hours are Monday-Friday 8:30 am - 5:00 pm.

Minimum Qualifications

• Knowledge of Health Education & Promotion: Understanding of population health data collection, community demographics, social/cultural environments, and chronic disease prevention. Ability to implement and evaluate health improvement programs and policies for diverse populations.
• Communication Skills: Excellent oral and written communication skills to effectively guide staff, educate internal/external stakeholders, and respond to data requests. Ability to develop and deliver training materials, seminars, and health education sessions.
• Resource Management: Knowledge of procurement processes, budget monitoring, and ensuring proper use of resources. Ability to prepare grant applications, manage grant activities, and maintain compliance with funding guidelines.
• Collaboration & Relationship Building: Ability to build and maintain effective working relationships with community partners, local health districts, and agencies to promote health initiatives and improve population health.
• Quality Improvement & Data Analysis: Skilled in analyzing and verifying data, identifying trends, and preparing reports to support health program planning. Ability to evaluate the success of health programs and make necessary adjustments.
• Technology Skills: Proficiency in using social media platforms for health communication, as well as experience with data management systems and office software (Word, Excel, PowerPoint).
• Project Management: Ability to develop and manage project plans, timelines, and resources to ensure successful implementation of health initiatives

Additional Considerations

Experience conducting CHA/CHIP activities
Experience leading or coordinating Community Health Assessment and/or Community Health Improvement Plans
Demonstrated experience in community engagement, coalition-building, or collaborative public health initiatives
Strong project management skills, including the ability to manage timelines, and deliverables
