Job Duties
Serves as the Healthy Communities Coordinator for the local health district's efforts to work on developing and implementing environmental strategies to improve population health. Represents the health district at community events and assists, as necessary in the coordinator of health community activities. Supports and reinforces healthy behaviors in the district by working with community coalitions, schools, businesses city leadership, urban designers and other local organizations and partners to build support for lifestyle change.
This is a restricted position, which is funded by the Title V grant for a year's period running July 2024 - June 2025. The availability of funding is scheduled for review annually and periodically thereafter. Continued employment is contingent on the continued availability of funds.
Minimum Qualifications
Knowledge of public health issues; public and private health care practices; program evaluation; planning and policy development; community organizing; survey methods and use of data and health education principles
• Skills in program planning; implementations and evaluation; community assessment and organization; writing reports and program plans; grant writing; public speaking; group dynamics and consultation with other professionals and the general public
Minimum KSAs:

A. Performance Management (for employees who supervise others)
• Serves in a managerial/supervisory capacity to conduct performance management activities and ensure onboarding/offboarding and training for staff
• Establishes and reviews work assignments and priorities and implements performance improvement strategies and/or problem resolution for related issues, in conjunction with program management and Human Resource staff
• Ensures relevant training and workplace safety for staff
• Conducts required performance monitoring and appraisals, establishes clear performance expectations, addresses deficiencies in a timely manner, and documents underperformance in accordance with state guidelines
• Supports employee growth through regular feedback, coaching, and professional development opportunities
B. Program Support
• Provides resources and connects patients to care and non-medical case management according to applicable standards and guidelines
• Encourages patients to attend medical appointments and remain adherent to their medication regimen; identifies barriers that may prevent the patient from attending appointments; links patients to additional services to reduce the barriers identified
• Works to strengthen relationships with community partners to ensure seamless referral processes and efficient identification of areas of concern
• Assists with coordination, implementation and completion of specific projects ensuring consistency with commitments and goals; participates in the implementation and management of project changes and interventions to achieve project outputs
• Maintains all documents required by VDH for case files and client interactions; enters all program related data into database
C. Assessments & Evaluations
• Collects and analyzes data for community needs assessments using a variety of established research methods, procedures, statistical formulas and techniques; verifies data for accuracy and identifies trends, develops projections and prepares reports as directed
• Obtains health-related data about social and cultural environments, social determinants of health, community assets, needs and interests from primary and secondary sources
• Determines needs for health programming/interventions including but not limited to education and services on the basis of obtained data
• Identifies opportunities for collaboration and enhances community and partner engagement in efforts to address health improvement priorities of the community at large
• Evaluates programmatic data and makes recommendations to improve program outcomes
D. Community Outreach
• Conducts community outreach to engage key partners in coalition activities and health improvement actions addressing priority health issues; responsible for organizing outreach activities in the community and attending outreach with local partners
• Serves as the primary contact for health fair and community event requests from public individuals, organizations and agencies; coordinates agency community event participation and manages use of outreach resources
• Community liaison for connecting community residents to public health programs, research interventions, and community assessments
• Advocates for community change through participation in city and statewide initiatives
E. Training
• Plans and conducts educational trainings to health professionals and clients, with established goals and objectives for each training
• Attends scheduled staff meetings; participates in in-services, workshops, and classes to maintain knowledge, skills and abilities; attends program-required training/educational sessions; identifies and requests attendance at appropriate educational programs to enhance job performance; maintains records of trainings and submits documentation to supervisor; shares information with colleagues
F. Administrative Support
• Assists management team(s) in monitoring and reporting project expenditures; utilizes assigned and allocated resources to efficiently carry out project objectives; makes recommendations to district management team(s) relative to potential budget needs or suggested program changes
• Requisitions equipment, supplies, and materials needed to carry out project initiatives; provides specifications for any special equipment or supplies with requisitions; maintains inventory of equipment
• Adequately maintains program databases and performs client file review; prepares requested reports for local, state and federal funding sources
• Assists with preparing the annual work plan and budget for contract renewal; completes narrative portions of the monthly report required for the grant
• Provides technical assistance statewide for program operation; assists customers in planning, promoting and conducting related health education
• Performs other related administrative and public health duties as assigned to support agency operations
Additional Considerations
• Certified Health Education Specialist (CHES) for Health Educators.
• Experience working in public health, social work, human services or health-related field preferred
