Job Duties

This position will be responsible for Performance Management Clinical Service & Oversight, Health Care Programs and Program Assessment.

Performance Management 

  • Serves in a managerial/supervisory capacity to conduct performance management activities and ensure onboarding/offboarding and training for staff
  • Establishes and reviews work assignments and priorities and implements performance improvement strategies and/or problem resolution for related issues, in conjunction with program management and Human Resource staff
  • Ensures relevant training and workplace safety for staff
  • Conducts required performance monitoring and appraisals, establishes clear performance expectations, addresses deficiencies in a timely manner, and documents underperformance in accordance with state guideline
  • Supports employee growth through regular feedback, coaching, and professional development opportunities

Clinical Service & Oversight

  • Schedules clinics and other public health activities
  • Schedules clinical staff and prioritizes the use of nursing resources in meeting demands for patient care and community needs
  • Assigns nursing staff for field activities
  • Provides cross training for nursing staff to support all programs in the clinical services.
  • Provides direct nursing services to clients when necessitated by scheduling needs or special considerations, including the administration of adult and childhood immunizations.
  • Assesses the need for in-services and schedules staff accordingly
  • Provides information and training to staff on new and existing policies and procedures.
  • Addresses client complaints and/or special needs
  •  Ensures that efficient and effective systems are in place to provide quality patient care in a patient/user friendly environment
  • Analyzes internal and external factors that may impact the delivery of essential public health services

Healthcare Programs

  • In cooperation and collaboration with the District Director, PH Nurse Manager, and District Leadership Team may plan, develop, organize, and direct nursing and public health care programs and activities.
  • Uses the Quad Council’s Core Community/Public Health Nurse Competencies (2018) as an evidence base for practice.
  • May represent District on various committees, task forces, and/or projects both within and outside the agency
  •  Works collaboratively with District healthcare programs to support population and public health program initiative
  •  Promotes population health by facilitating a holistic community health improvement process with community agencies/partners to achieve public health goals based on defined strategic plan, goals and objectives


Program Assessment

  • Assists in strategic planning process, assesses community health status, collects and analyzes data; implements and monitors science-based quality initiatives
  • Effectively assesses staff/community partner training/technical needs and designs/coordinates appropriate training presentation/materials
  •  Maintains current knowledge of practice in field of Communicable Disease/Population Health; participates in continuing education opportunities; meets continuing education requirements of the Virginia Board of Nursing
  • Use data related to the determinants of health and community resources to plan for, analyze, and evaluate community‐oriented and population‐level programs
  • Meets with grant accountant monthly to review IAP and TB budgets to ensure proper spending and management.
  • Works with Physician Supervisor, Business manager A to assess clinic capacity and plan how to improve flow and patient access.

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

Minimum Qualifications

  • Knowledge and demonstrated ability to effectively supervise staff in the areas of public health nursing and population health
  • Knowledge of grants, case management, ambulatory clinic services, and agencies/programs that support the underserved and underinsured populations
  • Knowledge of public and community health principles, and of nursing and health care programs
  • Knowledge of nursing theories related to improving the health of the community
  • Working knowledge of computer programs such as Microsoft Office Suite

Additional Considerations

  • Bi-lingual and experience with diverse cultures
  • Knowledge and experience working in a state and/or local health department or public health setting
  • Experience in developing and managing external outreach clinics
  • Strong knowledge management of clinic flow, practice management skills
  • In-depth understanding of clinic workflow optimization and operational management to ensure efficient patient care and resource utilization
