Job Duties
This position is considered essential personnel and would be required to work in the event of an emergency, pandemic, or facility need. All ESH positions are required to work different shifts to include nights, weekends and holidays, and/or overtime as requested. Under the direction of the Social Work Department Director, the Intensive Discharge Coordinator will provide oversight of discharge planning of ESH patients to include supervision of case managers, benefits coordinator and social work assistants, management of the discharge ready/extraordinary barriers list and direct oversight of the discharge planning progress for patients on the extraordinary barriers list. This position will liaison directly with CSB’s ,DBHDS central office staff and ESH leadership to monitor discharge progress, problem solve and escalate barriers as needed The ESH operational outcome for this position will be to reduce the length of stay of ESH patients on the extraordinary barriers list.
Minimum Qualifications
Master’s degree in social work is required.
Experience is needed in providing services to the mentally ill population and their families with an emphasis on discharge planning from a state psychiatric facility.
Valid driver’s license.
Additional Considerations
Licensed Master's in Clinical Social Work (LMCSW) is preferred.
Supervision experience is preferred.
