Job Duties

The Blue Ridge Health District (BRHD) is seeking a passionate Population Health Manager to join our team of dedicated public health professionals. BRHD serves the counties of Albemarle, Fluvanna, Greene, Louisa, Nelson, and the City of Charlottesville.

In this vital role, you will lead and coordinate health improvement planning efforts and communications across diverse communities, working collaboratively to improve population health outcomes.

What You’ll Do:
• Lead the development, implementation, and evaluation of the District's Community Health Assessment and Improvement Plan (CHA/CHIP).
• Deliver engaging health education programs and trainings for both the public and staff.
• Collect and analyze program data and prepare reports to inform decision-making.
• Develop and revise educational materials tailored to community needs and distribute them via electronic newsletters and social media.
• Serve as a public information officer (PIO), managing media relations, public communications, and social media.
• Provide technical assistance to local organizations and community partners.
• Be an active member of the BRHD Population Health Division, supporting other division programs including WIC, Community Health Workers, Maternal/Child Health programs, etc.
• May supervise staff and contribute to strategic planning efforts.

Minimum Qualifications

  • Considerable knowledge and experience in the principles and practices of health education, community health assessment and improvement planning, and community organization and engagement
  • Experience performing research and utilizing quantitative and qualitative methods for assessing data and programs
  • Experience with project management, development of goals and objectives, work planning, and coordination of activities and initiatives
  • Experience with grant management, grant writing, and grant activity reporting
  • Experience preparing and delivering media updates, informational materials, and presentations to a variety of audiences
  • Excellent communication skills, verbally and in writing, to effectively manage interpersonal and interorganizational communication and coordination networks
  • Proficiency in the use of standard Microsoft Office products

Additional Considerations

  • Previous experience in public health and the assessment/improvement process preferred
  • Certified Health Education Specialist (CHES) for Health Educators preferred
  • Experience with program management and activity reporting preferred.
  • Experience facilitating meetings and delivering presentations preferred.
  • Experience working across agencies, coalitions, or community organizations to align goals and drive collective impact preferred.
