Job Duties

This position provides supervisory leadership within the community nursing team and provides direct client care services for the long-term services and support (LTSS) screenings and Tuberculosis (TB) programs. Direct client care services included but are not limited to performing assessments, conducting home visits, case management for the LTSS and TB programs, and follow up on client referrals. Leadership responsibilities include performance management, program oversight, program evaluation, consultation, technical assistance, and caseload data tracking. This position also provides supervision and direct care services to cases involving elevated blood lead levels in children and will provide expert nursing interventions for human rabies prevention. Other duties of the position involve supporting community-based prevention efforts, participating in outreach and education initiatives,
working specialized vaccination events and serving as a key member of the district’s Epidemiological.

Response Team.
-In the event of an emergency declaration by the Governor, all public health nurses are required to assume other duties for the districts’ emergency preparedness and response (EP&R) activities, including but not limited to points-of-dispensing (POD) mass vaccination events and shelter coverage. During declared emergencies, this will require an adjustment to work hours, location, job duties as well as participation in trainings to meet public health business needs. Additional potential work-schedule variations include participation in community outreach events, participation in specialized district vaccine events, and other EP&R assignments, such as communicable disease and/or epidemiological investigation events as well as locality emergency shelter coverage.

VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

Minimum Qualifications

•Current RN license in the State of Virginia.

Knowledge and demonstrated ability to effectively supervise staff in the areas of public health nursing and population health.

• Knowledge of grants, case management, ambulatory clinic services, and agencies/programs that support the underserved and underinsured populations.

• Knowledge of public and community health principles, and of nursing and health care programs

• Knowledge of nursing theories related to improving the health of the community

• Working knowledge of computer programs such as Microsoft Office Suite
