Job Duties
This position leads as the subject matter expert for clinical coordination and discharge case management, ensuring compliance with State and Federal rules and regulations, accreditation standards, and internal policies. They will supervise the Clinical Coordinators and Discharge Coordinators, oversee processes, and provide cross-training to ensure continuous coverage of duties, including on-call.
Minimum Qualifications
Knowledge of clinical coordination and case management of patient continuum of care needs
Skilled in principles and practices of health care administration as well as clinical principles and practices
Demonstrated ability to communicate effectively orally and in writing throughout the organizational structure
Ability to interpret and follow oral and written policies/regulations
Experience working in a complex healthcare system
Previous supervisory experience
Additional Considerations
Case management certification
Correctional nurse certification
Current Virginia license in relevant field
Working knowledge of current ACA, NCCHC, and Joint Commission standards and compliance documentation
5 years relevant work experience
Previous correctional experience
